We empower individuals, families, caregivers, and communities by connecting them with the resources, guidance, and support they need to overcome barriers, strengthen well-being and move forward with confidence.

We envision communities where no one has to navigate life's journey alone.  Through the Journey Navigation Model, we are creating a future where healthcare, community resources, and people work together seamlessly....preserving each person's story, progress, and dignity through every stage of life.

Meet the Founder

Beth Patton, B.S.E, LMSW, CHW

Generations Community Health Connections, LLC was founded by Beth Patton, a licensed master's level social worker and Community Health Worker with more than 30 years of experience serving individuals and families across healthcare, behavioral health, education, disability services, aging, hospice, and community resource navigation.  

Throughout her career, Beth recognized that many people do not struggle because resources don't exist---they struggle because no one walks alongside them to connect the pieces.  That realization led to the development of Generations Community Health Connections and the Journey Navigation Model™ framework, designed to help individuals, families, healthcare providers, and community organizations move beyond referrals toward coordinated, person-centered support.

Her mission is simple:  help people navigate life's transitions with dignity while strengthening the partnerships that improve health outcomes across rural communities.

Community health navigation is built on relationships.  Every person, family, and community has a unique story, and meaningful support begins by listening, understanding barriers, and creating practical solutions together.  

We work alongside healthcare providers, caregivers, and community organizations to connect people with the resources they need while helping ensure those connections lead to lasting outcomes----not just referrals.
