Journey Pathwayâ„¢
Journey Pathway™ is the structured framework at the center of GCHC’s navigation approach. It helps move identified needs through screening, prioritization, coordinated action, follow-up, and resolution while keeping interconnected needs organized in one care plan.
Mapped Pathways
Journey Pathway™ includes 80+ mapped need and pathway options across 11 comprehensive screening domains. Pathways guide the worker through clear next steps while allowing the approach to adapt to each person’s needs and circumstances.
Training & Pilot Partnerships
GCHC collaborates with healthcare organizations, community partners, and local agencies to pilot, refine, and implement Journey Pathwayâ„¢ within real-world navigation and care coordination workflows.
6. RE-ENGAGE – Resume exactly where the work left off
A connected workflow that helps improve care continuity, reduce gaps, and keep unresolved needs from getting lost.
Why Journey Pathwayâ„¢ Is Different
More Than Navigation
Journey Pathwayâ„¢ does not stop with a referral or phone number. It supports movement toward resolution.
Handles Complexity
Multiple interconnected needs are organized, prioritized, and tracked in one connected care plan.
Actionable Next Steps
The worker can see what to do next, what has already happened, and what is still pending.
Continuity Over Time
Progress is saved so unresolved needs can be picked back up without starting over.
